Why this applies to Meloxicam

Meloxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Your body clears meloxicam mainly through a liver enzyme called CYP2C9. There's some thought that milk thistle might slow that enzyme down a bit. If it did, meloxicam could hang around longer and build up, which in theory could nudge up the usual NSAID side effects like stomach upset, heartburn, fluid retention, higher blood pressure, or in rare cases stomach irritation or kidney strain.

Here's the reassuring part: the evidence is shaky and conflicting. Some lab studies hint at this, but better human studies suggest milk thistle doesn't meaningfully block CYP2C9. The interaction is rated minor and unlikely. Still, it's worth a quick mention to your pharmacist or doctor, especially if you take meloxicam daily or have kidney or stomach concerns.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Meloxicam

What Is Meloxicam Used For?

Brand names: Mobic, Vivlodex, Qmiiz ODT, Anjeso

Meloxicam tablets and oral suspension treat the signs and symptoms of osteoarthritis, rheumatoid arthritis, and juvenile rheumatoid arthritis (JRA). The oral suspension and Zybic™ oral suspension are approved for JRA in children 2 years and older; meloxicam tablets are approved for JRA in patients who weigh at least 60 kg. Meloxicam capsules are approved specifically for the management of osteoarthritis pain.

Two IV (intravenous) formulations — QAMZOVA and XIFYRM — are indicated for adults for the management of moderate-to-severe pain, used alone or with non-NSAID pain relievers. Because these IV forms take time to reach full effect, they are not recommended when rapid pain relief is needed.
