Why this applies to Naproxen

Naproxen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist: your body uses a liver enzyme called CYP2C9 to help break down naproxen. There's a theory that milk thistle might slow that enzyme down a little. If it did, naproxen could clear a bit more slowly and hang around longer, which in theory could nudge up the usual NSAID side effects like stomach upset, heartburn, or, over time, irritation of the stomach lining.

The honest truth, though, is that the evidence here is shaky and conflicting. Some lab studies hint at it, but better human research suggests milk thistle doesn't meaningfully affect CYP2C9. So for most people taking naproxen, this is unlikely to cause a real problem. Still, it's worth a quick chat with your pharmacist, especially if you have any stomach or kidney concerns.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Naproxen

What Is Naproxen Used For?

Brand names: Naprosyn, Naprelan, Anaprox, Anaprox DS, Naproxyn, Navalbine, Naproxen, Naprosyn EC, Napro-Na, Aleve, Arthroxen, Synflex, EC-Naprosyn

Naproxen tablets, naproxen sodium tablets, and naproxen oral suspension are used to relieve the signs and symptoms of rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, and polyarticular juvenile idiopathic arthritis. They are also used for tendonitis, bursitis, and acute gout, and for managing pain and primary dysmenorrhea (menstrual cramps).

Naproxen Sodium Controlled-Release Tablets are indicated for the same conditions plus the relief of mild to moderate pain. Several over-the-counter naproxen sodium products are indicated for the temporary relief of minor aches and pains from headache, backache, muscular aches, toothache, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever.
