P-GLYCOPROTEIN SUBSTRATES

Diltiazem falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: milk thistle might nudge up how much diltiazem your body absorbs, because in lab studies it can block a transporter pump called P-glycoprotein. Diltiazem rides along on that pump, so blocking it could let a little more drug into your system. In theory, that could mean slightly stronger effects from diltiazem, like more dizziness, low blood pressure, a slower heartbeat, or swelling in your ankles.
But the real-world evidence says this is unlikely to cause problems. A human study with a similar P-glycoprotein medicine showed no meaningful change. So I'd file this under 'good to know,' not 'panic.' If you take both, just watch for new dizziness or a slow pulse and check in with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your liver uses an enzyme called CYP3A4 to break down diltiazem, and there's a theory that milk thistle might slow that enzyme down a little. If that happened, more diltiazem could linger in your body, which in theory could mean stronger effects like lower blood pressure, dizziness, a slower heartbeat, or swelling in your ankles.
The good news is that the evidence here is weak. This concern comes mostly from lab and test-tube studies, and actual human studies showed milk thistle did not meaningfully change how the body clears similar CYP3A4 drugs. So for most people taking diltiazem, this is unlikely to cause a problem. Still, it never hurts to mention milk thistle to your pharmacist or doctor, especially if you notice new dizziness.

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL
Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).
Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
