Fenfluamine falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might inhibit CYP2B6.
An in vitro study shows that silybin, a constituent of milk thistle, binds to and noncompetitively inhibits CYP2B6. Additionally, silybin might downregulate the expression of CYP2B6 by decreasing mRNA and protein levels.

Here's the gist of it: lab studies suggest milk thistle (specifically a compound called silybin) may slow down an enzyme called CYP2B6, which helps your body break down Fintepla (fenfluramine). If that enzyme is blocked, the medicine could clear out more slowly and build up a bit higher than expected. Fenfluramine is the active drug itself, not a prodrug, so more of it in the body could mean stronger effects and a higher chance of side effects like drowsiness, tiredness, loss of appetite, or changes in heart rate.

I want to be honest with you: this comes only from test-tube research, not real patient studies, so it's theoretical and may not happen at all. Still, because seizure control matters so much, please check with your child's neurologist or pharmacist before adding milk thistle.

What Is Fenfluramine Used For?

Brand name: Fintepla

Fintepla (fenfluramine oral solution) is approved to treat seizures associated with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It is used alongside other antiseizure medications.

Both conditions are severe, often hard-to-control forms of epilepsy that typically begin in childhood. Fintepla is available only through a restricted program called the Fintepla REMS because of serious heart-related risks.
