Irinotecan falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.

Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist with irinotecan (Camptosar). After your body uses this chemo drug, it gets cleared partly through a process called glucuronidation, which is handled by enzymes called UGTs. Lab studies suggest milk thistle's silymarin may slow those enzymes down. In theory, that could let irinotecan and its active form (SN-38) hang around longer than expected, which might bump up its side effects, things like serious diarrhea, low blood counts, infection risk, and nausea.

I want to be honest about the strength of this, though. This concern comes from test-tube and animal research, not from people, so it's more of a caution than a proven problem. Since irinotecan is potent and dosed carefully, please loop in your oncology team or pharmacist before adding milk thistle. Don't change anything on your own.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the good news first: when researchers actually measured this combination, milk thistle (in doses from about 420 to 1,350 mg a day) did not meaningfully change how your body handles irinotecan. So on paper, this is rated as a minor and unlikely interaction.

The theory is that milk thistle might slow the CYP3A4 enzyme that helps process irinotecan, which could let more of the drug build up and increase side effects like diarrhea, low blood counts, nausea, and fatigue. But that worry comes mostly from lab studies and a handful of case reports with other cancer drugs, not from solid human data on irinotecan itself. Because irinotecan is powerful chemo, please don't add milk thistle without first clearing it with your oncologist or pharmacist.

Irinotecan also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist for irinotecan (Camptosar). Milk thistle's silymarin can block a transporter called OATP-B1 that helps move certain drugs into your liver and tissues. Irinotecan relies on these uptake transporters, so in theory milk thistle could shift how much of the drug, and its active form SN-38, reaches the right places. That could nudge things either way, either toward more side effects like diarrhea and low blood counts, or toward less benefit.

Honestly, this is graded as a minor, mostly theoretical concern based on lab studies, with only a couple of loose reports involving other cancer drugs. Irinotecan is also given by IV in a clinic, so an oral absorption effect matters less. Still, since this is chemotherapy, please run any milk thistle use by your oncology team first.
