GLUCURONIDATED DRUGS

Apomorphine falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your apomorphine (Apokyn). Your body partly clears this medicine by attaching a sugar molecule to it, a process called glucuronidation that's handled by an enzyme called UGT. In test-tube studies, milk thistle's active compounds (silymarin) appear to slow that enzyme down. In theory, that could let apomorphine clear a little more slowly and build up slightly, which might mean stronger effects, more nausea or vomiting, sleepiness, dizziness, or low blood pressure when you stand.
I want to be honest with you though: this is based only on lab work, not on real people taking both together, so it's a possible concern rather than a proven one. Since apomorphine already causes nausea and blood pressure drops, it's worth a quick chat with your doctor or pharmacist before adding milk thistle. Don't change anything on your own.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the short version: this combination is very low on the worry list. Milk thistle's active part, silymarin, can mildly block a transporter called P-glycoprotein in lab studies. Apomorphine is carried by that same transporter, so in theory milk thistle could let a little more apomorphine into your system. In real life, though, this effect looks too small to matter.
The evidence backing any real-world problem is thin and mostly theoretical. If anything did happen, you might notice slightly stronger apomorphine effects like nausea, yawning, dizziness, lightheadedness when standing, or drowsiness. That's unlikely. Still, since apomorphine is a potent medicine for Parkinson's off episodes, it's worth mentioning milk thistle to your pharmacist or neurologist before you start it.

Apomorphine Injection treats “off” episodes in advanced Parkinson's disease. These are periods of poor movement and stiffness, sometimes called hypomobility. They include “end-of-dose wearing off” and unpredictable “on/off” episodes.
It is used for acute, intermittent treatment, meaning as-needed treatment of episodes as they happen. It has been studied as an add-on to other Parkinson's medicines.
