Theoretically, milk thistle might affect the clearance of drugs that undergo glucuronidation.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.

Here's the gist for milk thistle and tamoxifen. Your body partly clears tamoxifen by attaching a sugar molecule to it, a process called glucuronidation. In test tube studies, milk thistle's silymarin compounds slowed down the enzymes that do this. In theory, that could let a bit more tamoxifen stay in your system, which might mean slightly stronger effects or more of its usual side effects like hot flashes, leg cramps, or nausea.

I want to be honest, though: this is based only on lab work, not real people, so we don't know if it actually matters in your body. Tamoxifen is too important to gamble with, so please check with your oncologist or pharmacist before adding milk thistle. Don't change anything on your own.

Theoretically, the milk thistle constituent silibinin might increase tamoxifen levels and interfere with its conversion to an active metabolite.
Animal research suggests that the milk thistle constituent silibinin might increase plasma levels of tamoxifen and alter its conversion to an active metabolite. The mechanism appears to involve inhibition of pre-systemic metabolism of tamoxifen by cytochrome P450 (CYP) 2C9 and CYP3A4, and inhibition of P-glycoprotein-mediated efflux of tamoxifen into the intestine for excretion. Whether this interaction occurs in humans is not known.

Here's the tricky part with this pairing. Tamoxifen is actually a prodrug, which means your body has to convert it into its more active form (called endoxifen) before it does most of its work against breast cancer. In lab and animal studies, the silibinin in milk thistle seems to do two things: it may raise tamoxifen blood levels by slowing certain liver enzymes, but it may also interfere with that conversion into the active form. So the worry isn't more side effects, it's that you might end up with less of the active drug working for you.

I want to be honest though: this comes only from test-tube and animal research, so we don't know if it really happens in people. With a medicine this important, I'd play it safe. Please check with your oncologist or pharmacist before adding milk thistle.

In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's something worth knowing about tamoxifen. It's actually a prodrug, meaning your body has to convert it into its active form (called endoxifen) before it can fight breast cancer. Several liver enzymes help with that conversion, and CYP2C9 is one piece of the puzzle. The theory is that milk thistle might slow down CYP2C9, which in turn could nudge how much active drug you make.

I'll be honest with you, though: the evidence here is shaky and conflicting. Some lab studies hint at an effect, but careful human studies suggest milk thistle doesn't meaningfully block this enzyme. The risk is rated minor and unlikely. Still, because tamoxifen needs to work fully against cancer, I'd loop in your oncology pharmacist or doctor before adding milk thistle.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist for milk thistle and tamoxifen. Tamoxifen is partly moved around the body by a pump in your cells called P-glycoprotein. In a test tube, milk thistle can slow that pump down, and the theory is this might let a little more drug get absorbed. On paper, that could mean slightly stronger effects or side effects like hot flashes, leg cramps, or fluid changes.

In real life, though, this concern looks very weak. The interaction is rated minor and unlikely, and a human study with another P-glycoprotein drug (digoxin) showed milk thistle didn't really change its levels. So I wouldn't lose sleep over it. Still, since tamoxifen is a serious cancer medicine, please run any milk thistle plans past your oncologist or pharmacist first.

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist with milk thistle and tamoxifen. Your body uses certain liver enzymes, including CYP3A4, to handle tamoxifen. In theory, if milk thistle slowed that enzyme down, more tamoxifen could hang around and you might see more side effects like hot flashes, nausea, or leg swelling. But honestly, the evidence here is thin and conflicting. Some lab studies hint at it, while actual human studies showed milk thistle did not meaningfully change how similar drugs are cleared.

One wrinkle worth mentioning: tamoxifen is a prodrug, meaning it needs activation (mostly by a different enzyme, CYP2D6) to work fully. Because the overall picture is so uncertain, I'd just loop in your oncologist or pharmacist before adding milk thistle.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
