Diazepam falls under GLUCURONIDATED DRUGS — the group Milk Thistle interacts with.
Laboratory research shows that milk thistle constituents inhibit uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Other laboratory research suggests that a milk thistle extract of silymarin might inhibit beta-glucuronidase, although the significance of this effect is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here is the gist with milk thistle and your Valium. Diazepam goes through a couple of clean-up steps in the liver, and one of those steps is a process called glucuronidation. In test-tube and animal research, compounds in milk thistle (mainly silymarin) seem to slow that enzyme down a bit. In theory, that could let diazepam build up slightly, which might add to the drowsiness, dizziness, foggy thinking, or unsteadiness it can already cause.
I want to be honest about how soft this is. This is based only on lab and animal data, not real people, so it may not amount to much at all. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding milk thistle so they can keep an eye on you.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version: your body uses several liver enzymes, including one called CYP2C9, to break down diazepam (Valium). In theory, milk thistle might slow that enzyme a little. If it did, diazepam could clear a bit more slowly and add up in your system, which could mean extra drowsiness, grogginess, dizziness, or feeling unsteady on your feet.
But honestly, the evidence here is shaky and conflicting. Some lab studies hint at a slowdown, while actual human studies show milk thistle didn't meaningfully change levels of similar drugs. This interaction is rated minor and unlikely. Still, since diazepam can already make you sleepy, it's worth mentioning to your pharmacist or doctor, especially if you take a steady daily dose.

While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

But honestly, the evidence here is weak. Diazepam is not a prodrug, so we're only talking about mild buildup at most. Human studies using real milk thistle doses showed little to no effect on similar drugs, and this interaction is rated minor and unlikely. I wouldn't lose sleep over it, but since both can make you sleepy, it's worth a quick chat with your pharmacist or doctor, especially if you notice more grogginess.

Diazepam is used to relieve anxiety and to control agitation caused by alcohol withdrawal. It is also used along with other medications to control muscle spasms and spasticity caused by certain neurological disorders such as cerebral palsy (condition that causes difficulty with movement and balance), paraplegia (inability to move parts of the body), athetosis (abnormal muscle contractions), and stiff-man syndrome (a rare disorder with muscle rigidity and stiffness). Diazepam is also used along with other medications to control seizures. Diazepam is in a class of medications called benzodiazepines. It works by calming abnormal overactivity in the brain.
