Why this applies to Trimipramine

Trimipramine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Surmontil (trimipramine). Your body partly relies on a liver enzyme called CYP2C9 to break this antidepressant down. There's a theory that milk thistle might slow that enzyme a little. If it did, trimipramine could clear out more slowly and build up, which might mean more of the usual tricyclic side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.

The good news: the evidence here is shaky and the concern is rated minor and unlikely. Some lab studies hint at a small effect, but better human studies suggest milk thistle doesn't meaningfully block this enzyme. I wouldn't lose sleep over it, but since trimipramine affects the heart and brain, mention it to your pharmacist or doctor before combining them.

What Is Trimipramine Used For?

Brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp

Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.

In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
