Why this applies to Dapsone

Dapsone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for milk thistle and dapsone. Your body partly uses an enzyme called CYP3A4 to break dapsone down. In theory, milk thistle (its active part, silymarin) could slow that enzyme a little, which might let dapsone build up and raise the odds of side effects like headache, nausea, or, more seriously, blood-related problems such as anemia.

The good news is that this concern is pretty weak. It comes mostly from lab and animal work, and human studies show milk thistle usually doesn't change CYP3A4 drugs much. So a real problem with dapsone is unlikely. Still, since dapsone can stress red blood cells, it's worth mentioning milk thistle to your pharmacist or doctor before starting it.

Milk thistle is a popular herbal supplement most often used for liver health, and its main active component is a group of compounds called silymarin. While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Dapsone

What Is Dapsone Used For?

Brand names: Dapsone, Avlosulfon, Aczone

Dapsone tablets are used to treat dermatitis herpetiformis (a chronic, intensely itchy blistering skin condition) and leprosy (all forms, except where proven resistance to dapsone exists). For leprosy, dapsone tablets are typically used in combination with other anti-leprosy medicines.

Aczone Gel (dapsone gel, 7.5%) and dapsone gel are indicated for the topical treatment of acne vulgaris in patients 9 years of age and older. This topical form is applied to the skin and is not used for leprosy or dermatitis herpetiformis.
