Testosterone Undecanoate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: there's a theoretical worry that milk thistle could slow down an enzyme in your liver called CYP3A4, which helps process testosterone undecanoate. If that enzyme were blocked, testosterone could in theory build up a bit, which might mean more side effects like acne, mood changes, fluid retention, or a rise in red blood cell counts.

But honestly, the evidence here is weak and mostly from lab and animal work. Human studies actually showed milk thistle didn't meaningfully change how the body handles similar CYP3A4 medicines. So for most men this combo is unlikely to cause a real problem. Still, since your doctor monitors your testosterone levels and bloodwork anyway, it's worth mentioning milk thistle to your pharmacist or doctor so they know what you're taking.

About Testosterone Injection

What Is Testosterone Injection Used For?

Brand names: Andriol, Aveed, Jatenzo, Kyzatrex

Testosterone injection is primarily used in men whose bodies do not produce enough testosterone on their own — a condition called hypogonadism. This includes primary hypogonadism (a problem with the testicles themselves, such as from injury, infection, or surgery) and hypogonadotropic hypogonadism (caused by problems in the brain's hormone signaling from the hypothalamus or pituitary gland). It is also used to stimulate puberty in carefully selected adolescent males with clearly delayed puberty who have not responded to other support.

Testosterone Enanthate Injection is also approved for use in women with advanced, inoperable metastatic breast cancer (specifically skeletal metastases) who are one to five years past menopause, or in premenopausal women with hormone-responsive breast cancer who have had their ovaries removed. This use should be directed by an oncologist. Aveed (testosterone undecanoate) is specifically approved for male hypogonadism only.
