Orphenadrine falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might inhibit CYP2B6.
An in vitro study shows that silybin, a constituent of milk thistle, binds to and noncompetitively inhibits CYP2B6. Additionally, silybin might downregulate the expression of CYP2B6 by decreasing mRNA and protein levels.

Here's the simple version: your body uses an enzyme called CYP2B6 to help clear orphenadrine. Milk thistle (specifically a compound in it called silybin) may slow that enzyme down. If orphenadrine isn't broken down as quickly, more of it can linger in your system, which could nudge up its usual side effects, things like drowsiness, dry mouth, blurry vision, dizziness, or feeling lightheaded when you stand up.

That said, I want to keep this in perspective. This concern comes only from lab (test tube) research, not from real people, so it's theoretical and far from proven. Most folks won't notice anything. Still, if you take both and feel unusually sleepy or off, mention it to your pharmacist or doctor before changing anything.

Orphenadrine citrate is used to ease discomfort from acute, painful musculoskeletal conditions — things like muscle pain and stiffness. It is always used as part of a broader treatment plan that includes rest and physical therapy, not as a stand-alone cure.

Both the extended-release tablet and the injection form are approved for this use. Neither form directly relaxes tight muscles — the relief it provides is thought to be related to its pain-relieving (analgesic) properties.
