Ofloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Milk Thistle interacts with.

Milk thistle may inhibit one form of OATP, OATP-B1, which could reduce the bioavailability and clinical effects of OATP-B1 substrates.
In vitro research shows that milk thistle inhibits OATP-B1. Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are OATP substrates, including sorafenib and methotrexate. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist for ofloxacin and milk thistle. Milk thistle (its silymarin compounds) may block a transporter called OATP-B1 that helps your gut absorb certain drugs. In theory, that could let a little less ofloxacin get into your body, which might make the antibiotic slightly weaker against the infection you're treating. Ofloxacin is the active drug itself, not a prodrug, so this points toward a touch less benefit, not more side effects.

The good news: this is rated minor and rests only on lab and animal data, plus a couple of cancer-drug reports, not on ofloxacin specifically. I wouldn't lose sleep over it. Still, since you're fighting an infection, it's smart to run it by your pharmacist or doctor first.

About Ofloxacin

What Is Ofloxacin Used For?

Brand names: Oflox, Floxin, Floxin Injection, Floxin Otic

Ofloxacin is used in several different forms, each approved for specific infections. The otic (ear) solution treats outer ear infections (otitis externa) in patients 6 months and older, chronic middle ear infections with a ruptured eardrum (chronic suppurative otitis media) in patients 12 and older, and acute middle ear infections in children 1 year and older who have ear tubes. The ophthalmic (eye) solution — including the brand Ocuflox — treats bacterial conjunctivitis (pink eye) and bacterial corneal ulcers.

Ofloxacin oral tablets treat adults with mild to moderate bacterial infections including community-acquired pneumonia, skin and skin structure infections, gonorrhea, chlamydial infections of the urethra or cervix, mixed urethral/cervical infections, and acute pelvic inflammatory disease. The oral tablet is also used for acute bacterial exacerbations of chronic bronchitis and uncomplicated cystitis, but only when no other treatment options are available, due to serious risk concerns with this drug class.
