Why this applies to Nifedipine

Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Your body relies on an enzyme called CYP3A4 to break down nifedipine. The thinking here is that milk thistle might slow that enzyme down a little. If it did, nifedipine could hang around longer than usual, which in theory might lead to more of its typical effects, like a bigger drop in blood pressure, dizziness, flushing, headache, or swelling in the ankles.

Here's the reassuring part: the actual human studies don't really back this up. When people took milk thistle alongside other CYP3A4 drugs, their levels didn't change much. So this is a minor, unlikely concern based mostly on lab work. Still, if you notice extra lightheadedness, mention it to your pharmacist or doctor before changing anything.

While it is generally well tolerated, the scientific evidence that it meaningfully improves liver disease or other conditions is mixed and far from conclusive. About Nifedipine

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
