Why this applies to Temazepam

Temazepam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for Restoril (temazepam): in theory, milk thistle could slow one of the liver enzymes (CYP3A4) that helps break this sleep aid down. If that happened, temazepam might hang around a little longer, which could mean more next-day grogginess, drowsiness, or feeling unsteady on your feet.

The good news is that the evidence here is pretty weak. It comes mainly from lab and animal studies, and human studies with similar drugs didn't show a real change. Temazepam also isn't a prodrug, so it doesn't need to be activated to work. For most people this is unlikely to be a problem, but if you notice extra morning sleepiness, mention it to your pharmacist or doctor.

Temazepam capsules — sold as Restoril and under the generic name Temazepam — are indicated for the short-term treatment of insomnia, generally 7 to 10 days. They help people fall asleep faster and stay asleep longer.

This medication is intended for short periods only. If sleep trouble doesn't improve within 7 to 10 days, that may signal an underlying medical or psychiatric condition that needs its own evaluation.
