Why this applies to Prednisolone

Prednisolone falls under P-GLYCOPROTEIN SUBSTRATES — the group Milk Thistle interacts with.

Theoretically, milk thistle might increase the absorption of P-glycoprotein substrates. However, this effect does not seem to be clinically significant.
In vitro research shows that milk thistle can inhibit P-glycoprotein activity and 1 case report from the World Health Organization (WHO) adverse drug reaction database describes increased abdominal pain in a patient taking milk thistle and the cancer medication vincristine, a P-glycoprotein substrate, though this patient was also taking methotrexate. However, a small pharmacokinetic study in healthy volunteers shows that taking milk thistle (Enzymatic Therapy Inc.) 900 mg, standardized to 80% silymarin, in 3 divided doses daily for 14 days does not affect absorption of digoxin, a P-glycoprotein substrate.

Here's the gist of it: milk thistle (specifically its silymarin compounds) might gently slow down a transporter in your gut called P-glycoprotein, which helps move certain drugs around. Prednisolone is one of the medicines handled by that transporter, so in theory milk thistle could let a little more of it get absorbed.

The good news is that this is mostly a lab-based idea. In real people, studies of similar P-glycoprotein drugs (like digoxin) showed no meaningful change, and the evidence for any problem is weak. If anything were to shift, you'd watch for slightly stronger steroid effects (think trouble sleeping, mood swings, fluid retention, or stomach upset), but that's unlikely. I wouldn't lose sleep over this combo. Still, run it by your pharmacist or doctor first, especially since you may be on prednisolone for an important reason.

Prednisolone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down prednisolone, and milk thistle has been looked at as something that might slow that enzyme down. If it did, prednisolone could linger a bit longer and you might notice more of its usual effects, like fluid retention, higher blood sugar, mood or sleep changes, or stomach upset.

The good news is that the evidence here is weak and conflicting. Careful human studies found milk thistle did not meaningfully change how the body handles similar CYP3A4 drugs, so a real problem is unlikely. Still, since everyone's situation differs, it's worth a quick word with your pharmacist or doctor before pairing the two, especially if you take prednisolone long term.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
