Why this applies to Dextromethorphan

Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for milk thistle and your cough medicine. The worry on paper is that milk thistle might slow down a liver enzyme called CYP3A4, which is one of the pathways that helps clear dextromethorphan. If that enzyme were blocked, the drug could linger a bit longer and you might notice more of its usual effects, like drowsiness, dizziness, or feeling a little foggy.

The good news is that this is very much a theoretical concern. Studies in people show milk thistle does not meaningfully change CYP3A4 activity, and the rating here is minor and unlikely. For a short course of cough syrup, I wouldn't lose sleep over it. Still, if you take milk thistle daily or feel unusually groggy, mention it to your pharmacist.

What Is Dextromethorphan Used For?

Brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
