Tisotumab Vedotin-Tftv falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the short version: the worry is that milk thistle might block an enzyme called CYP3A4, which helps clear the cell-killing part of Tivdak from your body. If that part lingers longer than it should, you could in theory see more of this drug's typical side effects, things like eye irritation or vision changes, nerve tingling or numbness in your hands and feet, nausea, bleeding, and fatigue.

I'll be honest, the evidence here is thin. Lab and animal studies conflict, and human studies haven't shown a clear problem. A handful of case reports raised flags with certain cancer drugs, so caution makes sense. Since you're getting cancer treatment, please check with your oncology team or pharmacist before adding milk thistle.

  • Tisotumab vedotin-tftv injection is used to treat cervical cancer (cancer that begins in the opening of the uterus [womb]) that has not improved or has come back after treatment with other medications or has spread to other parts of the body. Tisotumab vedotin-tftv is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
