Why this applies to Lidocaine

Lidocaine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist for milk thistle and lidocaine. Lidocaine is partly broken down by a liver enzyme called CYP3A4, and the worry on paper is that milk thistle (its silymarin compounds) might slow that enzyme down. If that happened, lidocaine could hang around longer and theoretically build up. But lidocaine is an active drug, not a prodrug, so it wouldn't lose its effect, the concern would be the opposite.

The good news: this is a weak, mostly theoretical interaction. Human studies show milk thistle doesn't meaningfully change CYP3A4 drugs, and most lidocaine people use is a cream, patch, or gel where very little reaches the bloodstream. Realistically the risk is low. Still, if you use injected lidocaine or take other medicines, mention the milk thistle to your pharmacist or doctor.

About Lidocaine Transdermal Patch

What Is Lidocaine Transdermal Patch Used For?

Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido

Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.

Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
