Why this applies to Lidocaine

Lidocaine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group 1,3-dmaa interacts with.

1,3-DMAA strongly inhibits cytochrome P450 2D6 (CYP2D6) enzymes in vitro. Theoretically, 1,3-DMAA can increase levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the short version: DMAA appears to block an enzyme called CYP2D6, and lidocaine is partly cleared through that enzyme. In theory, slowing that breakdown could let more lidocaine build up in your body. Lidocaine isn't a prodrug, so more drug around means stronger effects, not weaker ones. At high enough levels, lidocaine can cause dizziness, numbness or tingling around the mouth, ringing in the ears, a slow or irregular heartbeat, and in serious cases seizures.

That said, this is based only on lab (in vitro) testing, so it's theoretical. Most over-the-counter lidocaine creams and patches put very little into your bloodstream. Honestly, the bigger concern is DMAA itself, which is banned and unsafe. Please don't use it, and check with your pharmacist if you have questions.

What Is Lidocaine Transdermal Patch Used For?

Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido

Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.

Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
