Grapefruit and St. John's wort both act on CYP3A4, the liver enzyme that clears lidocaine, one raising blood levels and the other cutting them, while fever bark may destabilize the heart during general anesthesia. Since lidocaine numbs tissue and, given intravenously, steadies dangerous heart rhythms, a shifted level can mean toxicity in one direction or lost effect in the other.
Of the remaining 142 entries, 111 rate moderate and 31 minor, and most work the same enzyme angle, nudging CYP3A4 or CYP2D6 activity up or down. Six ingredients can also lower the seizure threshold, which matters because seizures are a hallmark of lidocaine overdose.
