CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Lidocaine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.
In vitro research shows that BMPEA strongly inhibits CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for lidocaine. Your liver uses an enzyme called CYP2D6 to help break it down. In lab testing, BMPEA appears to block that enzyme pretty strongly. If it does the same thing in people, your body could clear lidocaine more slowly, letting more of it build up. Lidocaine is not a prodrug, so more drug simply means stronger effects and a higher chance of side effects like dizziness, drowsiness, numbness around the mouth, ringing in the ears, or in rare cases heart rhythm or blood pressure changes.
That said, this is theoretical, based only on lab data, not real patient cases. Skin patches and creams put very little lidocaine into your blood, so the risk there is small. Still, BMPEA itself is an unapproved stimulant best avoided. Please check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your liver uses an enzyme called CYP3A4 to help clear lidocaine. BMPEA may slow that enzyme down a bit, so in theory more lidocaine could hang around in your body. If levels climbed too high, you might notice things like dizziness, a numb or tingling mouth, drowsiness, or an unusual heartbeat.
I want to keep this in perspective, though. This concern is theoretical, coming from lab (test tube) data showing only partial enzyme blocking, not from real patient cases. Most over-the-counter lidocaine is a patch or cream that barely reaches your bloodstream, so the risk there is low. That said, BMPEA is a risky, unregulated stimulant I'd steer clear of. What Is Lidocaine Transdermal Patch Used For?
Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido
Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.
Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
