Why this applies to Lidocaine

Lidocaine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist with shiitake-derived lentinan and lidocaine. Lidocaine is partly cleared by a liver enzyme called CYP2D6, and lab studies hint that lentinan might speed that enzyme up a bit. In theory, a faster enzyme could break lidocaine down quicker, which might leave a little less of it around and slightly weaken its pain relief. Lidocaine is active as-is (not a prodrug), so faster clearance means a weaker effect, not a stronger one.

I want to be honest about the strength here: this is only seen in test tube and animal research, and it has never actually been reported in people. For a lidocaine cream, patch, or gel you rub on the skin, very little reaches the bloodstream, so I wouldn't expect a real problem. Still, if you're using lidocaine and considering lentinan, run it past your pharmacist or doctor first.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the basic idea: your liver uses an enzyme called CYP3A4 to help break down lidocaine, and lab studies hint that lentinan might speed that enzyme up a bit. If that happened in real life, your body could clear lidocaine a little faster, which might mean slightly less pain relief from it. Lidocaine isn't a prodrug, so faster breakdown means weaker effect, not stronger.

About Lidocaine Transdermal Patch

What Is Lidocaine Transdermal Patch Used For?

Brand names: Xylocaine, Dilocaine, Lidoject-1, Xylocaine MPF, Nervocaine, Lidoject-2, Duo-Trach Kit, Octocaine, Lidoderm, Ztlido

Lidocaine skin patches are applied directly to the painful area to treat post-herpetic neuralgia (PHN) — the persistent burning or aching nerve pain that can remain after a shingles outbreak.

Lidoderm (lidocaine patch 5%) and ZTlido (lidocaine topical system 1.8%) are both indicated for this use in adults. Note that despite their different strengths, one ZTlido topical system provides equivalent lidocaine exposure to one Lidoderm patch.
