Lentinan and Temazepam

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Temazepam

Temazepam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Temazepam (Restoril). Your liver uses an enzyme called CYP3A4 to help clear this sleep medicine. Lab and animal studies hint that lentinan might speed that enzyme up. If that happened in people, your body could break temazepam down a little faster, which in theory might make it work less well, so you could notice it doesn't help you fall or stay asleep as reliably.

I want to be honest though: this is only test-tube and animal evidence, and it has never actually been reported in humans, so I wouldn't lose sleep over it. Temazepam isn't activated by this enzyme, so it's not a prodrug situation. Still, mention the lentinan to your pharmacist or doctor so they can keep an eye on how well your sleep is controlled.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Temazepam page

“Alcohol is the interaction I would flag first for anyone taking temazepam (Restoril), a benzodiazepine sleep medicine. Beer and wine both carry major ratings because they layer their own sedation on top of a drug that already depresses the nervous system, and that additive drowsiness runs through 78 of the 197 supplements on file.”
