Lentinan + Erythromycin
Erythromycin brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, and others.
Lentinan has a moderate interaction with Erythromycin. Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4).

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Erythromycin
Erythromycin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears erythromycin partly through a liver enzyme called CYP3A4. Lab research hints that lentinan might rev up that enzyme. If it sped things up in real life, your body could break down erythromycin a bit faster, which in theory might lower its levels and make it less effective against the infection you're treating.
I want to keep this in perspective, though. This is based only on test-tube and animal work, and it has not been seen in actual patients. So it's more of a theoretical heads-up than a proven problem. Still, if you're on erythromycin for an active infection and thinking about adding lentinan, run it by your pharmacist or doctor first so nothing undercuts your treatment.

Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Erythromycin page
“Erythromycin calls for more supplement caution than most antibiotics, and the reason is one liver enzyme, CYP3A4. Erythromycin both blocks that enzyme and is broken down by it, so grapefruit can push erythromycin levels up while St.”

About Lentinan
What is Lentinan?
- Immune support
- Used alongside cancer treatment
- Anti-inflammatory
- General wellness/tonic
- Antimicrobial support
Lentinan is a beta-glucan polysaccharide from shiitake mushrooms that is studied mainly as an immune-modulating add-on to cancer chemotherapy, where it is given by injection in a few countries. Evidence for oral supplements is limited, and quality varies widely between products. Talk with your pharmacist or doctor before using it, especially if you have a health condition or are pregnant or breastfeeding.
About Erythromycin
What Is Erythromycin Used For?
Brand names: Erythro S, PCE, E-Mycin, Erythro-ES, EryPed Chewable, Eryc, Ery-Tab, Rythro Enca, Erythro E-C, Erythro Base, Ilosone, Ilotycin
Erythromycin treats infections caused by susceptible bacteria. The oral products (delayed-release tablets, tablets and suspensions) are labeled for mild to moderate upper respiratory tract infections caused by Streptococcus pyogenes, Streptococcus pneumoniae and Haemophilus influenzae. For H. influenzae, it is used together with adequate doses of sulfonamides.
Erythromycin lactobionate injection (Erythrocin Lactobionate and other lactobionate products) is used for similar infections, including lower respiratory tract infections, when oral treatment is not possible or when the infection needs high blood levels right away. Topical erythromycin 2% solution, Ery 2% pads and Spotex gel are used on the skin to treat acne vulgaris.

Erythromycin Is Linked to Lower Levels of 7 Nutrients
Natural Medicines records Erythromycin as a medicine that can lower these. This is not an interaction with Lentinan — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
