Why this applies to Naloxone, Pentazocine

Naloxone, Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Talwin Nx combines pentazocine, a pain reliever, with naloxone, which is added to discourage misuse. Pentazocine is partly handled by a liver enzyme called CYP1A2. The thought here is that lentinan, a shiitake mushroom extract, might speed that enzyme up a little. If that happened, your body could clear pentazocine a bit faster, which in theory might mean weaker pain control.

Here's the honest part: this is based only on lab and animal studies, not on real people, so I wouldn't lose sleep over it. Food amounts of shiitake mushrooms are very unlikely to matter; concentrated supplements are the bigger question mark. If you're using both, just mention it to your pharmacist or doctor and let them know if your pain relief seems to fade.
