Why this applies to Diltiazem

Diltiazem falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for diltiazem: your liver uses an enzyme called CYP3A4 to break it down. Lab and animal research hints that lentinan might rev up that enzyme a bit. If that happened in people, your body could clear diltiazem faster, leaving less of it working. That could mean your blood pressure or chest pain (angina) might not stay as well controlled as it should.

I want to be honest about the evidence though. This is only test-tube and animal data, and it has never been reported in actual patients. So I wouldn't lose sleep over it. Just keep an eye on your blood pressure readings and any returning chest tightness, and check with your pharmacist or doctor before adding lentinan rather than changing anything on your own.

Oral extended-release diltiazem (capsules and tablets) treats high blood pressure (hypertension). It is also used for chronic stable angina, which is chest pain from the heart working harder than its blood supply allows. It can be used alone or with other blood pressure medicines.

Some extended-release capsule products, such as Cartia, Cartia Xt and certain generic diltiazem capsules, are also approved for angina due to coronary artery spasm. The intravenous (IV) injection is a different use: it gives temporary control of a rapid heart rate in atrial fibrillation or atrial flutter, and rapidly converts paroxysmal supraventricular tachycardia (PSVT) to normal rhythm.
