Why this applies to Ivabradine

Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the short version with ivabradine. Your body relies on an enzyme called CYP3A4 to break down this medication. Lab research hints that lentinan might speed up that enzyme, which could clear ivabradine out faster and leave less of it working in your system. Ivabradine isn't a prodrug, so faster clearance simply means a weaker effect.

In practice, that could mean your heart rate slows down less than your doctor intended, which is the whole point of taking it for heart failure. I want to stress this is only theoretical right now, seen in lab studies and never confirmed in people, so don't be alarmed. Still, please loop in your pharmacist or doctor before pairing the two so they can keep an eye on how you're responding.

Lentinan is a beta-glucan polysaccharide from shiitake mushrooms that is studied mainly as an immune-modulating add-on to cancer chemotherapy, where it is given by injection in a few countries. Evidence for oral supplements is limited, and quality varies widely between products. About Ivabradine

What Is Ivabradine Used For?

Brand names: Procoralan, Coralan, Corlentor, Corlanor

Ivabradine (including the brand Corlanor) is used in adults with stable, symptomatic chronic heart failure with reduced pumping function to reduce the risk of being hospitalized for worsening heart failure. It is prescribed when the heart is beating too fast at rest and the patient is either already on the highest tolerated dose of a beta-blocker or cannot take one.

Corlanor is also approved to treat stable, symptomatic heart failure caused by dilated cardiomyopathy (an enlarged, weakened heart muscle) in children ages 6 months and older who have an elevated heart rate.
