Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for your cough medicine. Your body uses an enzyme called CYP2D6 to break down dextromethorphan. Lab research hints that lentinan might speed up that enzyme. If that happened in people, your body could clear the dextromethorphan a bit faster, so it might not quiet your cough as well as usual.

I want to be honest about how thin this evidence is. It comes only from test-tube and animal studies, and nothing like it has been reported in real patients. Dextromethorphan is also a short-term, over-the-counter medicine, so the practical risk is small. If you notice your cough isn't easing the way you'd expect, just check in with your pharmacist or doctor.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for your cough medicine. Your body partly clears dextromethorphan using a liver enzyme called CYP3A4. Lentinan, the shiitake mushroom extract, has shown in lab and animal studies that it may speed up that enzyme. If it sped things up in people too, your body could break down dextromethorphan a little faster, which in theory might make your cough syrup wear off sooner or feel slightly less effective.

I want to be straight with you, though: this is only test-tube and animal evidence, and it has never actually been seen in humans. Dextromethorphan is also a gentle, short-term cough remedy, so I wouldn't lose sleep over it. Still, if you take both and notice your cough isn't easing up, mention it to your pharmacist or doctor.

What Is Dextromethorphan Used For?

Brand names: Hold, Benylin Adult, Benylin Pediatric, Theraflu Thin Strips Long Acting Cough, Neo Citran Thin Strips Cough

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
