Why this applies to Cyclosporine

Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the thing to know about cyclosporine: your body relies on an enzyme called CYP3A4 to break it down. Lab studies hint that lentinan might speed that enzyme up, which could clear cyclosporine out of your system a little faster and lower its blood levels. Cyclosporine isn't a prodrug, so faster clearance means less drug working, not more.

Why does that matter? Cyclosporine has a narrow safety window, especially for transplant patients. If levels drop too low, you risk weaker protection against organ rejection or a flare of psoriasis or arthritis. That said, this is only test-tube and animal data, not something seen in real people, so don't panic. Please loop in your transplant team or pharmacist before adding lentinan, since they can check your levels.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
