Why this applies to Doxepin

Doxepin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for doxepin. Your body uses a liver enzyme called CYP2D6 to break it down. Lab research hints that lentinan might rev up that enzyme a bit, which could clear doxepin out faster and lower its levels in your blood. If you're taking the Silenor form for sleep, that might mean it doesn't help you stay asleep as well as it should.

The important part: this has only been seen in test-tube and animal studies, never in real people, so it's more of a theoretical concern than a proven problem. Doxepin is not a prodrug, so any effect would be a weakened one, not stronger side effects. Still, before you pair these, check in with your pharmacist or doctor so they can keep an eye on how well your medicine is working.
