Tofacitinib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the gist for Tofacitinib (Xeljanz). Lab studies hint that lentinan may speed up an enzyme called CYP2C19, which is one of the pathways your body uses to clear tofacitinib. If that enzyme works faster, your body could break down tofacitinib a bit quicker, leaving less of it around. In plain terms, that could mean your medicine works less well, so your rheumatoid arthritis, psoriatic arthritis, or ulcerative colitis symptoms (joint pain, swelling, or flares) might not stay as controlled.

I want to be honest about the evidence: this is only seen in test tube and animal research, and it has not been reported in real people. So it's a theoretical concern, not a proven problem. Tofacitinib isn't a prodrug, so the worry is reduced effect, not toxicity. Still, chat with your pharmacist or doctor before adding lentinan so they can keep an eye on how you're doing.

Tofacitinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist of it. Your body partly clears tofacitinib (Xeljanz) through a liver enzyme called CYP3A4. Lab studies hint that lentinan might speed that enzyme up. If that held true in people, it could mean your body breaks tofacitinib down a little faster, leaving less of it working in your system. For an arthritis or colitis medicine, weaker drug levels could mean your joint pain, swelling, or gut symptoms aren't as well controlled.

That said, I don't want to overstate this. The concern comes only from test-tube and animal research, and it has never actually been reported in humans, so it's mostly theoretical. Still, before pairing lentinan with your tofacitinib, run it by your pharmacist or doctor so they can keep an eye on how well you're doing.

What Is Tofacitinib Used For?

Brand names: Xeljanz, Xeljanz XR

  • Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
