Why this applies to Dorzolamide, Timolol

Dorzolamide, Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the thing about your Cosopt eye drops: the timolol part is a beta-blocker that the liver clears partly through an enzyme called CYP2D6. In test-tube studies, lentinan seems to speed up that enzyme a little, which in theory could lower timolol levels and make it work a bit less well. Since you put these drops right in your eye for glaucoma, the goal is keeping your eye pressure controlled.

I want to be honest though: this is only lab-dish research, and it has never actually been reported in real people, so I wouldn't lose sleep over it. The amount of timolol that even reaches your bloodstream from eye drops is small. Just mention the lentinan to your eye doctor or pharmacist, and keep up with your scheduled pressure checks.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
