Phenytoin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for phenytoin (Dilantin). Your body partly clears phenytoin through a liver enzyme called CYP2C19. Lab research hints that lentinan might speed that enzyme up, which in theory could clear phenytoin a little faster and pull its blood level down. Lower levels could mean your seizure control slips, so the worry here is reduced effect rather than too much drug.
I want to be honest about the evidence: this comes only from test-tube and animal studies and has not been seen in real patients. Still, phenytoin is a narrow-margin medication where even small swings matter, so please don't change anything on your own. Loop in your pharmacist or doctor before adding lentinan, and they can check a phenytoin level if needed.

What Is Phenytoin Used For?

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.
Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
