Formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the honest picture for your Formoterol inhaler. The concern on file is that lentinan, a shiitake mushroom extract, might speed up an enzyme called CYP2C19. When that enzyme runs faster, drugs it handles can get cleared from the body a little quicker.

The good news is this is really a theoretical worry. It comes only from lab research, it has not been seen in real patients, and formoterol isn't a drug that leans heavily on CYP2C19 for its breathing benefit. So in plain terms, I wouldn't expect your inhaler to suddenly stop controlling your wheezing or shortness of breath because of lentinan. Still, if you notice your COPD symptoms creeping back, mention it to your pharmacist or doctor before changing anything.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for your inhaler. Some of the formoterol your body absorbs is broken down by a liver enzyme called CYP2D6. Lab studies hint that lentinan might speed that enzyme up a bit, which in theory could clear formoterol a little faster and make it slightly less effective at keeping your airways open. In real life that might look like more wheezing, chest tightness, or shortness of breath sneaking back in.

I want to be honest, though: this is only test-tube and animal data, and it has never been reported in actual people. So I wouldn't lose sleep over it. If you do want to try lentinan, just give your pharmacist or doctor a heads-up and keep an eye on your breathing.

About Formoterol Oral Inhalation

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
