Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for rifampin: lab research hints that lentinan may speed up a liver enzyme called CYP3A4, which is one of the pathways that helps clear rifampin from your body. If that happens, your body could break rifampin down a little faster, leaving slightly less of it around to fight the bacteria. With a TB or meningitis-prevention medicine, you really want full strength on board so the infection is properly knocked out.

I want to be honest about how shaky this is. It comes only from test-tube and animal work, and it has never actually been seen in people. So this is more a theoretical caution than a proven problem. Don't stop or change anything on your own. Just give your pharmacist or doctor a heads-up before adding lentinan.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
