Rifampin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Apple interacts with.

Concomitant consumption of apple juice can significantly decrease oral absorption and blood levels of OATP substrates.
Research shows that consuming apple juice inhibits OATP, which reduces bioavailability of oral drugs that are substrates of OATP. Fexofenadine, atenolol, and aliskiren are substrates of OATP. Clinical research shows that coadministration of apple juice decreases bioavailability of fexofenadine by up to 78%, aliskiren by 63%, and atenolol by up to 82%. These effects appear to increase with larger quantities of apple juice. It is thought that apple juice might affect OATP for only a short time. Therefore, separating drug administration and consumption of apple juice by at least 4 hours might avoid this interaction.

Here's the gist: rifampin gets into your body partly through tiny transporters in your gut called OATP, and apple juice can temporarily block those gateways. When that happens, less of your dose actually gets absorbed, so your blood levels drop. With an antibiotic like rifampin, weaker levels are a real concern, because under-treating tuberculosis can let the bacteria survive and even become resistant. That's not a side effect you'd feel, which makes it sneaky.

The good news is the timing fix is simple. This blocking effect seems short-lived, so keeping your rifampin and apple juice at least a few hours apart usually sidesteps the problem. A regular apple as food is far less concentrated and unlikely to be the same issue. Please run your routine past your pharmacist or doctor before changing anything.

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.

It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.

Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
