Spironolactone falls under ANTIHYPERTENSIVE DRUGS — the group Apple interacts with.
Some clinical evidence suggests that consuming apple and cherry juice can increase blood pressure in elderly patients.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: spironolactone (Aldactone) is a water pill that lowers blood pressure and helps your body hold onto potassium. The concern on file isn't about whole apples you eat as a snack, it's about drinking apple juice. A lower-quality study suggested that apple and cherry juice could nudge blood pressure upward in older adults, which would work against what your spironolactone is trying to do.
So you might notice your blood pressure running a bit higher than expected if you drink a lot of apple juice regularly. Eating a fresh apple now and then is very unlikely to cause trouble. If you take spironolactone for blood pressure or heart failure, it's worth mentioning your juice habits to your pharmacist or doctor, and keep checking your readings at home.

What Is Spironolactone Used For?
Brand names: Aldactone, Spiroton, Carospir
Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.
Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
