Why this applies to Lovastatin

Lovastatin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Apple interacts with.

Concomitant consumption of apple juice can significantly decrease oral absorption and blood levels of OATP substrates.
Research shows that consuming apple juice inhibits OATP, which reduces bioavailability of oral drugs that are substrates of OATP. Fexofenadine, atenolol, and aliskiren are substrates of OATP. Clinical research shows that coadministration of apple juice decreases bioavailability of fexofenadine by up to 78%, aliskiren by 63%, and atenolol by up to 82%. These effects appear to increase with larger quantities of apple juice. It is thought that apple juice might affect OATP for only a short time. Therefore, separating drug administration and consumption of apple juice by at least 4 hours might avoid this interaction.

Here's the gist for your cholesterol pill. Apple juice can briefly block a gut transporter called OATP that helps certain medicines get absorbed. Lovastatin partly relies on this kind of uptake, so drinking apple juice around the same time could lower how much of the drug actually gets into your system. That means it may not lower your cholesterol as well as it should, which is the whole point of taking it.

Honestly, the strongest data is on drugs like fexofenadine and atenolol, not lovastatin specifically, so this is a reasonable concern rather than a proven big problem. Eating a regular apple is unlikely to matter much. If you drink a lot of apple juice, an easy fix is to space it at least 4 hours from your lovastatin. Check with your pharmacist or doctor before changing anything.

What Is Lovastatin Used For?

Brand names: Mevacor, Altocor

Lovastatin tablets are used along with a low saturated fat, low cholesterol diet when diet and other non-drug measures alone haven't done enough. The goal is to lower total and LDL cholesterol and reduce the risk of atherosclerotic vascular disease (cholesterol-related artery disease).

Lovastatin tablets are used to reduce the risk of heart attack, unstable angina and coronary revascularization procedures in people without symptomatic heart disease. They also slow the progression of coronary atherosclerosis in people with coronary heart disease. They treat primary hypercholesterolemia (high cholesterol) and are used in adolescents aged 10 to 17 with heterozygous familial hypercholesterolemia who meet certain criteria.
