Lovastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the deal with lovastatin and grapefruit. Your body clears lovastatin using a liver enzyme called CYP3A4. Grapefruit blocks that enzyme, so the drug isn't broken down as quickly and can build up to higher than expected levels in your blood. Lovastatin is one of the statins most sensitive to this, so this is a pairing I take seriously.
When levels climb too high, the main worry is muscle trouble: aching, soreness, or weakness, and rarely a serious breakdown of muscle that can harm the kidneys. Even a glass or two of grapefruit juice can have an effect that lingers for a day or more. Please don't stop either one on your own. Talk with your pharmacist or doctor about whether you should avoid grapefruit or switch to a different statin.

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)
2 of 3Lovastatin also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).
In vitro and clinical research show that consuming grapefruit juice inhibits OATP, which reduces the bioavailability of oral drugs that are substrates of OATP. Various clinical studies have shown reduced absorption of OATP substrates when taken with grapefruit, including fexofenadine, acebutolol, aliskiren, celiprolol, levothyroxine, nadolol, and pitavastatin. Grapefruit juice is thought to affect OATP for only a short time. Therefore, separating drug administration and consumption of grapefruit by at least 4 hours is likely to prevent this interaction.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the wrinkle with lovastatin and grapefruit. The interaction on file is about an absorption transporter called OATP, where grapefruit can actually lower the amount of certain drugs that get absorbed. But lovastatin has a second, much bigger issue: it's heavily broken down by a gut enzyme called CYP3A4, and grapefruit strongly blocks that enzyme. In real life, that blocking effect tends to win out, so grapefruit usually raises lovastatin levels in your body.
Why that matters: more lovastatin floating around can mean a higher chance of muscle aches, weakness, and (rarely) serious muscle breakdown. Concentrated juice matters more than a single bite of fruit. Please don't change anything on your own, just ask your pharmacist or doctor how to space or handle grapefruit safely.

Clinical research shows that grapefruit juice inhibits metabolism and increases absorption and plasma concentrations of statins that are metabolized by CYP3A4. These include lovastatin, simvastatin, and atorvastatin. Keep in mind that there is considerable variability in the effect of grapefruit juice on drug metabolism, so individual patient response is difficult to predict.
Some statins, including pravastatin, fluvastatin, pitavastatin, and rosuvastatin, are not metabolized by CYP3A4. However, grapefruit juice might still affect the bioavailability of these statins. These statins are substrates of OATP. Grapefruit juice can inhibit OATP. Therefore, grapefruit juice may reduce the bioavailability or increase drug levels of these statins depending on the type of OATP. However, grapefruit juice affects OATP for only a short time. Therefore, separating drug administration by at least 4 hours is likely to avoid this interaction.

Here's the deal with grapefruit and lovastatin: your body relies on a liver enzyme called CYP3A4 to clear this statin out of your system. Grapefruit blocks that enzyme, so the drug doesn't break down the way it should. The result is more lovastatin hanging around in your blood than your dose intended, and lovastatin happens to be one of the statins most strongly affected.
Why does that matter? Higher levels raise your odds of statin side effects, especially muscle aches, weakness, and (rarely) a more serious muscle problem that can stress the kidneys. About Lovastatin
What Is Lovastatin Used For?
Brand names: Mevacor, Altocor
Lovastatin is used to lower high LDL ("bad") cholesterol and total cholesterol in people whose levels have not improved enough with diet alone. It is also used to slow the buildup of plaque in the arteries (coronary atherosclerosis) in people who already have coronary heart disease.
In adults without existing heart disease but with high cholesterol and low HDL ("good") cholesterol, lovastatin tablets are approved to reduce the risk of heart attack, unstable angina, and the need for procedures to open blocked arteries. Lovastatin tablets are also approved for adolescents aged 10–17 with a specific inherited condition called heterozygous familial hypercholesterolemia (heFH), when diet changes alone haven't been enough.
