Fluvastatin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Apple interacts with.

Concomitant consumption of apple juice can significantly decrease oral absorption and blood levels of OATP substrates.
Research shows that consuming apple juice inhibits OATP, which reduces bioavailability of oral drugs that are substrates of OATP. Fexofenadine, atenolol, and aliskiren are substrates of OATP. Clinical research shows that coadministration of apple juice decreases bioavailability of fexofenadine by up to 78%, aliskiren by 63%, and atenolol by up to 82%. These effects appear to increase with larger quantities of apple juice. It is thought that apple juice might affect OATP for only a short time. Therefore, separating drug administration and consumption of apple juice by at least 4 hours might avoid this interaction.

Here's the gist for fluvastatin (Lescol): your body relies on a transporter called OATP to help shuttle this cholesterol medicine from your gut into your bloodstream. Apple juice can temporarily block that transporter. In studies with other OATP drugs, apple juice cut how much got absorbed by a lot, sometimes more than half. So drinking apple juice close to your dose could mean less fluvastatin gets in, which may make it work less well at lowering your LDL ('bad') cholesterol over time.

This is mainly about juice, not an apple you eat now and then. To stay safe, a simple fix is spacing apple juice and your fluvastatin by at least 4 hours. If you drink it daily, mention it to your pharmacist or doctor before changing anything.

About Fluvastatin

What Is Fluvastatin Used For?

Brand names: Lescol, Lescol XL

Fluvastatin capsules are used alongside diet to lower total cholesterol, LDL cholesterol, triglycerides, and apolipoprotein B, and to raise HDL cholesterol in adults with primary hypercholesterolemia (high cholesterol) or mixed dyslipidemia (abnormal cholesterol and triglyceride levels). Fluvastatin capsules are also used in boys and girls aged 10–16 who have heterozygous familial hypercholesterolemia (an inherited form of high cholesterol) when diet alone has not worked. In adults with clinically evident coronary heart disease (CHD), fluvastatin capsules may reduce the risk of needing a coronary revascularization procedure (a procedure to restore blood flow, such as angioplasty) and can slow the progression of coronary atherosclerosis (plaque buildup in heart arteries).

Fluvastatin sodium extended-release tablets and Lescol XL (extended-release tablet) share similar uses: reducing LDL cholesterol in adults with primary hyperlipidemia, reducing LDL in adults and children aged 10 and older with heterozygous familial hypercholesterolemia who require the highest available dose, and reducing cardiovascular risk in adults with established coronary heart disease. Fluvastatin is always used together with a cholesterol-lowering diet, not as a replacement for one.
