Here's the practical takeaway: aliskiren (Tekturna) lowers your blood pressure, and there's some clinical evidence that drinking apple juice (and cherry juice) could nudge blood pressure up in older adults. That means apple juice might work against what your medication is trying to do, leaving your readings a little higher than expected. This isn't about aliskiren building up or being cleared faster, it's about the juice itself possibly affecting your blood pressure control.

Honestly, eating a regular apple as food is a different story and shouldn't be a worry. The concern is more with drinking apple juice. If you enjoy apple juice often, just mention it to your pharmacist or doctor and keep an eye on your home blood pressure readings.

Concomitant consumption of apple juice can significantly decrease oral absorption and blood levels of aliskiren.
Pharmacokinetic research shows that coadministration of apple juice 200 mL along with aliskiren 150 mg decreases the bioavailability of aliskiren by 63%. Apple juice seems to inhibit organic anion transporting polypeptide (OATP), which is involved in drug uptake in the gut, liver, and kidney. It is thought that apple juice might affect OATP for only a short time. Therefore, separating drug administration and consumption of apple juice by at least 4 hours might avoid this interaction.

Here's something worth knowing if you take aliskiren (Tekturna) for blood pressure. Apple juice can actually block how much of this medicine your body absorbs. In one study, drinking about a cup of apple juice with a dose cut the amount that got into the bloodstream by roughly 63 percent. That's a big drop. The juice temporarily slows a gut transporter (called OATP) that helps pull aliskiren into your system, so less of the drug makes it in.

The practical worry is weaker blood pressure control, meaning your numbers may run higher than expected. The good news: this effect is short-lived, so spacing your apple juice and your aliskiren dose by at least 4 hours usually avoids the problem. Eating a whole apple is far less concerning than drinking the juice. Check with your pharmacist if you drink apple juice regularly.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
