Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice can increase levels of drugs metabolized by CYP3A4.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.

Here's the short version with aliskiren (Tekturna): your body partly clears this blood pressure medicine through an enzyme called CYP3A4, and grapefruit slows that enzyme down. With the cleanup crew working slower, more aliskiren can hang around in your system than expected. That usually means a stronger blood pressure lowering effect, which can leave you feeling dizzy, lightheaded, or tired, especially when you stand up quickly.

One thing to know: grapefruit's effect can linger for up to two days, so spacing it out from your dose doesn't really fix it. This matters more if you're older or take several medicines. Please don't change anything on your own. Just check with your pharmacist or doctor about whether grapefruit fits with your routine.

Grapefruit juice can decrease blood levels of aliskiren, potentially decreasing the clinical effects of aliskiren.
Clinical research shows that grapefruit juice can decrease aliskiren levels by approximately 60% by inhibiting organic anion transporting polypeptide (OATP). Grapefruit juice is thought to affect OATP for only a short time. Therefore, separating drug administration and consumption of grapefruit by at least 4 hours is likely to prevent this interaction.

Here's something a bit counterintuitive about aliskiren (Tekturna) and grapefruit. With many medicines, grapefruit makes drug levels go up. With aliskiren, it's the opposite. Grapefruit juice blocks a transporter called OATP that helps carry aliskiren into your body, so less of the drug gets absorbed. Studies have shown drops of around 60 percent in blood levels.

In real terms, that means your aliskiren may not lower your blood pressure as well as it should, which could leave your numbers higher than your doctor wants. The good news is this effect is short-lived. Spacing your grapefruit and your dose by at least 4 hours usually avoids the problem. Talk with your pharmacist or doctor about timing before changing anything.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
