Segluromet combines two medicines (ertugliflozin and metformin) that work together to lower your blood sugar. Neither one is a prodrug, so this isn't about your body activating the drug. Instead, the concern here is simpler: apple juice can carry a fair amount of natural sugar, and drinking a lot of it could nudge your blood sugar up, which works against what your medication is trying to do.

Keep in mind this is theoretical, based on the sugar content rather than any direct chemical clash. A whole apple has fiber that blunts the sugar hit, so it's much gentler than juice. Enjoy apples as a normal part of your diet. If you drink juice often or notice your numbers running high, mention it to your pharmacist or doctor.
