Lentinan + Dextromethorphan, Guaifenesin, Psuedoephedrine
Dextromethorphan, Guaifenesin, Psuedoephedrine brand name: Aquatab C.
Dextromethorphan, Guaifenesin, Psuedoephedrine falls into 2 drug categories that Lentinan can interact with; the most serious is rated moderate. Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6).

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Lentinan interacts with whole categories of medication. Dextromethorphan, Guaifenesin, Psuedoephedrine falls into 2 of those categories, so it can interact with Lentinan in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Dextromethorphan, Guaifenesin, Psuedoephedrine
Dextromethorphan, Guaifenesin, Psuedoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Out of the three ingredients in Aquatab C, only dextromethorphan (the cough suppressant) is really affected here. Your body uses an enzyme called CYP2D6 to clear it. Lab research hints that lentinan might speed that enzyme up, which could clear dextromethorphan a little faster and make it slightly less effective at quieting your cough. The guaifenesin and pseudoephedrine parts aren't broken down this way, so they shouldn't change.
Here's the reassuring part: this is only seen in test-tube and animal studies, and it has never been reported in actual people. So I wouldn't lose sleep over it. Since Aquatab C is usually short-term, any effect is likely small. Still, it's worth a quick chat with your pharmacist if you plan to use lentinan regularly.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan, Guaifenesin, Psuedoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your Aquatab C, which combines dextromethorphan (the cough suppressant), guaifenesin (loosens mucus), and pseudoephedrine (the decongestant). Lab studies hint that lentinan might speed up an enzyme called CYP3A4. Of these three ingredients, dextromethorphan leans partly on that pathway, so in theory a faster enzyme could clear it a little quicker and make it slightly less effective for quieting a cough.
I want to be honest about how soft this is. The signal comes only from test tube and animal research, and it has never actually been seen in people. Guaifenesin and pseudoephedrine aren't really driven by this enzyme anyway. I wouldn't lose sleep over it, but if you're using lentinan regularly, just run it by your pharmacist or doctor first.
