Dextromethorphan HBr, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for your cough and chest formula. The dextromethorphan part of this product is cleared by a liver enzyme called CYP2D6. Lab studies hint that lentinan might rev up that enzyme a bit. If that happened, your body could clear dextromethorphan faster, which might make the cough-calming effect a little weaker than usual.

I want to be honest about the strength of this, though. It comes only from test-tube and animal research, and it has never actually been reported in people. The guaifenesin piece isn't affected at all. For a short-term cough remedy, I wouldn't lose sleep over it, but if you take lentinan regularly, mention it to your pharmacist so they can keep an eye on things.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the short version for your cough syrup. Lab studies hint that lentinan, a beta-glucan from shiitake mushrooms, might speed up an enzyme called CYP3A4. Dextromethorphan (the cough suppressant) is partly handled by this enzyme, so in theory a faster enzyme could clear it a bit quicker and make it slightly less effective at quieting a cough. The guaifenesin part, which loosens chest mucus, isn't really driven by this pathway, so it shouldn't be affected.

I want to keep this in perspective. This is only test-tube and animal data, and it has never actually been seen in people. For an over-the-counter cough product, I wouldn't lose sleep over it. If you use lentinan regularly and notice your cough medicine working less well, just check in with your pharmacist.
