Dextromethorphan, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your cough and chest combo. The dextromethorphan part is cleared by an enzyme called CYP2D6, while guaifenesin (the mucus thinner) mostly stays out of this. Lab studies hint that lentinan might rev up CYP2D6 a bit. If that happened in people, your body could break down dextromethorphan a little faster, which might make the cough-quieting effect feel weaker than usual.
I want to be honest though: this is only seen in test tube and animal research, never reported in actual patients, so it's very theoretical. Practically, I wouldn't lose sleep over it. If your cough just isn't easing up while you're taking lentinan, mention it to your pharmacist or doctor and we can sort it out together.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for your cough and chest combo. The dextromethorphan part is handled in your body partly by an enzyme called CYP3A4 (and also CYP2D6). Lab research hints that lentinan may speed CYP3A4 up a bit. If that happened, your body could clear dextromethorphan a little faster, which might make the cough-suppressing effect feel weaker than usual. The guaifenesin piece isn't really tied to this enzyme, so it's mostly unaffected.
The good news is this is only test-tube and animal evidence, with nothing yet seen in real people, so I wouldn't lose sleep over it. Still, if your cough medicine doesn't seem to be working while you're on lentinan, mention it to your pharmacist or doctor before changing anything.
