Here's the simple version: tragacanth is mostly a gel-forming fiber, and when it sits in your stomach and gut, it can grab onto pills and change how much medicine your body soaks up. With tofacitinib (Xeljanz), that could mean you absorb a little less, a little more, or no different at all. This isn't a problem with how tofacitinib is broken down, it's purely about timing in the gut.

I want to be honest that this is theoretical, based on how fiber behaves rather than real studies in people, so I wouldn't lose sleep over it. Tofacitinib does matter for keeping your arthritis or colitis controlled, so the easy fix is spacing them out. A good rule is to take your tofacitinib first and wait about 30 to 60 minutes before the tragacanth. If you're unsure, your pharmacist can confirm the timing for your routine.

Tragacanth is a plant gum used mainly as a thickener and emulsifier in foods, cosmetics, and medicines, and traditionally to soothe the throat and gut. There is very little solid human research showing it treats any specific condition. It is generally considered safe in food amounts, but anyone with allergies or who is pregnant or breastfeeding should check with a pharmacist or doctor first.

What Is Tofacitinib Used For?

Brand names: Xeljanz, Xeljanz XR

  • Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
