Tofacitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP3A4 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help clear tofacitinib (Xeljanz) out of your system. Some Acacia rigidula products have been found spiked with a stimulant called BMPEA, and in lab tests that compound slowed CYP3A4 down by about a third. If that happened in a person, less of the drug would be broken down, so tofacitinib levels could creep up. With an active medication like this, more drug can mean a higher chance of side effects such as infections, headaches, or changes in your blood counts and cholesterol.

I'll be honest, this is theoretical and only comes from test-tube research, so I wouldn't panic. But Acacia rigidula has real safety concerns of its own. Please check with your pharmacist or doctor before combining the two.

Acacia rigidula is a shrub from Texas and Mexico that is marketed in weight-loss and energy supplements. About Tofacitinib

What Is Tofacitinib Used For?

Brand names: Xeljanz, Xeljanz XR

Tofacitinib is used in adults with moderately to severely active rheumatoid arthritis, active psoriatic arthritis, active ankylosing spondylitis, and moderately to severely active ulcerative colitis. In each case it is for people who did not respond well to, or could not tolerate, one or more TNF blockers.

Xeljanz and tofacitinib tablets and oral solution are also used in children 2 years and older with active psoriatic arthritis or active polyarticular course juvenile idiopathic arthritis (arthritis in multiple joints). Extended-release tablets are labeled for adults only.
