Here's the short version for tofacitinib (Xeljanz). When your liver turns adrafinil into modafinil, that modafinil can slow down an enzyme called CYP2C19, which is one of the pathways your body uses to clear tofacitinib. If that pathway gets blocked, a little more tofacitinib may hang around than expected.

In theory, that could nudge up tofacitinib's effects and its side effects, things like a higher infection risk, headaches, or changes in lab values. I want to be honest though: this is a theoretical concern based on lab data, not real patient cases, so I wouldn't panic. Since tofacitinib affects your immune system, it's still worth a quick chat with your pharmacist or doctor before combining them.

Here's the basic idea with these two. Once your liver turns adrafinil into modafinil, that modafinil can act like a mild accelerator on a liver enzyme called CYP3A4. Tofacitinib (Xeljanz) is partly cleared by that same enzyme. So in theory, speeding it up could lower your tofacitinib levels a bit and make it work less well, meaning your rheumatoid arthritis, psoriatic arthritis, or ulcerative colitis symptoms (joint pain, swelling, stiffness, or gut flare-ups) might not be as well controlled.

I want to be honest, though: this is a theoretical concern based on how the drugs behave, not solid patient data, and modafinil is only a weak inducer. So the real-world effect is likely small. Still, before combining them, please check in with your doctor or pharmacist.

What Is Tofacitinib Used For?

Brand names: Xeljanz, Xeljanz XR

  • Tofacitinib is used alone or with other medications to treat rheumatoid arthritis (condition in which the body attacks its own joints causing pain, swelling, and loss of function) in adults who are unable to take or did not respond to one or more tumor necrosis factor (TNF) inhibitor medication(s). It is also used to treat psoriatic arthritis (a condition that causes joint pain and swelling and scales on the skin) in adults who are unable to take or did not respond to or tolerate one or more TNF inhibitor medication(s). Tofacitinib is used to treat ulcerative colitis (a condition which causes swelling and sores in the lining of the colon [large intestine] and rectum) in adults who are unable to take or who did not respond to one or more TNF inhibitor medication(s). It is also used to treat polyarticular juvenile idiopathic arthritis (PJIA; a type of childhood arthritis that affects five or more joints during the first six months of the condition, causing pain, swelling, and loss of function) in children 2 years and older who are unable to take or who did not respond to one or more TNF inhibitor medication(s). Tofacitinib is in a class of medications called Janus kinase (JAK) inhibitors. It works by decreasing the activity of the immune system.
